A reflection on
diabetes care in Brunei
Eighteen months ago, the author was working as a Diabetes Nurse Specialist at Frimley Park Hospital and just completing her advanced certificate in diabetes care. After her husband was posted with the army as Senior Medical Officer to the British garrison in Brunei Darussalam (Brunei), however, the author took up a role as a Practice Nurse in the Medical Reception Station there. 
She wondered whether she would be able to use her knowledge and skills out in the Far East. In this article, the author reflects on her experience of a diabetes clinic in Brunei for use as a learning tool.

What is insulin resistance?

 

In the liver insulin resistance increases glucose output and impairs insulins ability to inhibit liver glucose production, a process known as gluconeogenesis. What this means is that the liver keeps producing glucose (aka sugar) even if blood sugar levels are already high and this only causes more insulin to be produced, increasing insulin resistance.

Insulin resistance also occurs in fat cells. Fat cells don’t contribute to high blood sugar but high insulin in the blood promotes more fat storage which can contribute to weight gain and continue the vicious cycle.
